Responsibilities of the Travel Case Manager Nurse: Coordinate and
oversee the patient's continuum of care across various healthcare
settings Conduct comprehensive assessments and develop

transitions Monitor patient progress and modify care plans as
needed to achieve clinical and financial goals Provide education

Registered Nurse (RN) license Current BLS certification Acute care
experience is required Minimum of 2 years Case Manager experience
